If you would like to be considered for financial support, please
complete the following form. If this form cannot be submitted from
your browser, send an e-mail with the same information to Michal Misiurewicz.
We will not be able to determine the exact level of support until after
the Conference is over, but we will provide in advance lower bounds on the
amount of support we can provide. If you submit a request by September 19, we
will respond by September 26. Requests for support can also be made at
the Conference, but it is not clear how much money will remain by
that time.
If you receive support from us, you will be reimbursed after the
Conference. You will need to submit receipts for everything.
If you use an electronic ticket, please request a receipt from the
airline. A copy of an e-mail from a travel agency is not sufficient.
